Form 1095-C Availability Notice (2025 Reporting Year)

Restaurant Life LLC d/b/a Galuppi’s provides Form 1095-C in accordance with IRS requirements under the Affordable Care Act (ACA).

For the 2025 reporting year, the IRS allows Applicable Large Employers (ALEs) to furnish Form 1095-C upon employee request, rather than automatically mailing the form to all employees.

How to Request Your Form 1095-C
Employees and former employees may request a copy of their Form 1095-C at no cost by submitting the form on this page.

Upon request, Form 1095-C will be provided no later than March 2, 2026, as required by IRS regulations.

This notice will remain available on our website through October 15, 2026.

When You Will Receive the Form

Requests will be fulfilled as quickly as possible. In all cases, Galuppi’s will provide the Form 1095-C no later than March 2, 2026, in accordance with IRS requirements.
